Should HSE departments report directly to CEOs to strengthen accountability?

I have seen some organisational structures where the HSE department is managed by the HR Head or Operations Head.  I find it difficult to believe that such HSE departments can be effective because they are reporting to the same units they are supposed to monitor. I believe the only way HSE can be taken seriously in any organisation is to have the department report directly to the CEO or MD.
